INSTRUCTIONS ON THE EXERCISE OF THE CONSUMER’S RIGHT TO WITHDRAW FROM A DISTANCE CONTRACT AND A CONTRACT CONCLUDED OUTSIDE THE TRADER’S BUSINESS PREMISES

Right to Withdraw from a Service Contract

You have the right to withdraw from this contract without giving any reason within 14 days. The withdrawal period will expire 14 days from the date of its conclusion (the contract is deemed to be concluded on the date the service provider confirms the service order). When exercising your right to withdraw from the contract, please inform us of your decision to withdraw from this contract by an unequivocal statement sent electronically to the email address app@healthprofile.digital

For this purpose, you may use the model withdrawal form that we have provided or sent to you; however, its use is not mandatory.

The withdrawal period is deemed to be complied with if you send the notice of exercising the right to withdraw from the contract before the withdrawal period expires. Please note that you do not have the right to withdraw from the contract once the provision of the services has commenced.

Consequences of Withdrawal from the Contract

By withdrawing from the contract, the contract is terminated. After withdrawal from the contract, we will refund all payments you have made in connection with the conclusion of the contract, except for the costs of delivering the goods to you. The payments will be refunded no later than 14 days from the date on which we receive your notice of withdrawal from this contract. The refund will be made using the same means of payment as you used for the initial transaction, unless you have expressly agreed otherwise, and no additional fees will be charged.
